HAZARDOUS MATERIALS AND WASTE - ARCHIVED

Biennial Hazardous Waste Reports are Due March 1, 2002!!

The US Environmental Protection Agency (EPA) requires all Large Quantity Generators (LQG's) of hazardous waste and all treatment, storage, and disposal facilities (TSDFs) to submit a Biennial Hazardous Waste Report by March 1, 2002. For an extension you must contact your State or Regional Office by February 23, 2002. The Biennial Hazardous Waste Report consists of: a Site Identification Form which includes general information pertaining to the facility; generator status; and on-site waste management status; Form GM that is required to be completed for each hazardous waste which was generated on-site, managed on-site or shipped off-site during the reporting year; Form WR if, during the reporting year, it received hazardous waste from off-site sources; and Form OI if required by your State. The Site identification form has been revised, and there are new Source and Form codes on the GM form.
